In 1983 Massachusetts’ enacted Chapter 688, which is better known as the “Turning 22” law.
This provided a planning process for the transition of young adults with severe disabilities and extensive medical needs into the adult service system after high school graduation or upon turning 22 years old. This community capstone project entails participation in the opening of a five-person community residence for medically fragile individuals. This home will provide an opportunity for five individuals who have aged out of the pediatric facility to transition into a community supported residential home. Working with key stakeholders with interagency collaboration this home will provide residential supports including coordination of medical and non-medical services. This medical home will strive to provide a comprehensive model of care that is individualized, compassionate, care-coordinated, and culturally effective with the greatest possibility of autonomy as community members.
